An Enhanced Model of Primary Care for Individuals with Mental Illness and Addictions

With the movement toward the integration of primary and behavioral health care, enhanced levels of primary care may be necessary for individuals living with mental illness and addictions. Life expectancy and other outcomes for patients with serious mental illness (SMI) are unacceptably poor, largely due to a high prevalence of poorly-controlled chronic diseases.  However, it is less well  understood how to deliver effective primary care for patients living with mental illness (SMI) and addictions. Join this webinar to hear from two integrated care medical providers on a proposed framework for enhanced primary care delivery in integrated care settings. Participants should have a basic understanding of integrated primary and behavioral health.
